St. Joan de Déu Hospital in Barcelona was dealing with inefficient processes, a growing patient backlog and the inability to scale care for children with type 1 diabetes. Without standardized remote monitoring systems, staff time was stretched thin, and well-managed patients were using in-person slots that could be better allocated to those who truly needed support. Leadership aligned around the goal to become a Center of Excellence in pediatric diabetes care. To achieve this, they partnered with Medtronic IHS to implement the Diabeter Services solution.
A technology-enabled Integrated Practice Unit (IPU) model was deployed, combining multidisciplinary in-clinic care with digital clinical tools and remote monitoring technologies. This included VCare for disease management, CloudCare® for population oversight and Pathway Optimization to improve care efficiency. The results were transformative: ketoacidosis hospitalizations dropped by 58%, emergency visits decreased by 60% and HbA1c levels improved alongside a 25.8% increase in Time in Range. Clinicians saw a 43% reduction in face-to-face visits.
